Patient Forms
At ClearCut Rehab, we want to maximize your time with us. If you would like to save time on your first visit, please complete the Online Patient Intake Form and submit that online. Then, print the New Patients Forms below, fill them out and bring them with you to your first appointment. Otherwise, allow 15-20 minutes of extra time before your appointment to fill them out here.
Direct Access
Did you know you don’t need a referral to see a physical therapist?
Not everyone is aware that if you are experiencing pain or an injury, you can go directly to your physical therapist. It makes no difference whether you’re dealing with an old or new problem. Direct access allows you to see your physical therapy specialist right away without seeing a physician first for a designated amount of time. Please contact us today for more information on how long you can attend physical therapy sessions before needing a physician’s referral.
Please note, some insurances require a referral to get started. It can typically come from any type of physician, but if your insurance is an HMO plan then it will need to come directly from your primary care provider.
Direct Access in the medical community refers to the ability to see a physical therapist whenever you want without obtaining permission from your physician or medical practitioner.

We gladly accept the insurance plans listed below. If your plan is not listed, please call us at 817-719-7714 as we are always adding new insurance plans.
As a courtesy to our patients, we will verify your health benefits prior to your arrival to determine if there will be a copay, deductible, or coinsurance for therapy services.
